Currently, Dr. Ko is working on a number of highly important research projects intended to find and develop new cancer treatments for patients. One of her projects involves the study of a potentially more effective treatment for XXXXX Melanoma, the most serious form of skin cancer. The number of new cases of melanoma diagnosed each year in the United States has more than doubled in the past three decades and continues to grow. However, for some people, a combination of genetics, environment, and lifestyle can significantly increase the risk of developing melanoma. This form of cancer is usually treated with surgical removal of the primary tumor. At present, DTIC and high-dose IL-2 (a chemical of the immune system that usually works to stimulate white blood cells (lymphocytes) to fight infection) are the only FDA approved treatments for stage IV melanoma. Dr. Ko is presently conducting a phase II trial using extended-dosing temozolomide in patients with advanced melanoma. At the present time no chemotherapy or immunological therapy has a single agent activity against melanoma greater than 15%. This phase II trial will test the hypothesis that extended-dosing temozolomide (a chemotherapeutic agent) results in a 30% response rate. If true, this will make extended-dosing temozolomide the most active single agent in melanoma and would change the standard of care. Moreover, there are other important correlative studies relating to this trial. In particular, the trial will test the hypothesis that methylation of the MGMT promoter on the genes of the tumor cells correlates with response to the chemotherapy. Dr. Ko postulates that the changes in the DNA of some tumors are responsible for their resistance to chemotherapy. This implies that one could test tumors for DNA changes in their genetic code prior to administering chemotherapy. This will allow physicians to know beforehand whether chemotherapy is likely to work on a particular patient. Further, the trial will also test the hypothesis that temozolomide causes T-cell dysfunction leading to an immunocompromised state, causing lymphopenia and opportunistic infections in cancer patients. The chemotherapeutic agent temozolomide causes the white blood cells of the cancer patient to not function properly, compromising the immune system and making the patients susceptible to infections. Another important research project conducted by Dr. Ko involves immunotherapy for melanoma patients who have been rendered free of the disease by surgery but who are still at risk of recurrence. Dr. Ko was a critical member of the team of medical specialists who led in the development of innovative adjuvant (preventive) immunotherapy strategies intended to reduce the risk of cancer recurrence and prolong survival. Dr. Ko’s team was among the first to develop and study monoclonal antibodies and vaccines that might help prevent the spread or recurrence of a tumor in melanoma patients. The team also pioneered types of DNA vaccines in the laboratory to trigger immune responses to melanoma. Dr. Ko is also performing critical medical research in regard to the treatment of a rare type of cancer called sarkoma. Soft-tissue sarcomas arise in tissues such as fat, muscles, nerves, tendons, and blood and lymph vessels – the soft tissues that connect, support, and surround other parts of the body. Soft-tissue sarcomas present unique challenges in detection and treatment. Sarcomas are unusual in that they can occur in any site of the human body, although about half occur in the limbs. Her study will establish a database to show which chemotherapy regimen was used and what the outcome of the treatment was. The results will then help to establish a standard of care not presently available in this rare disease. Finally, Dr. Ko is conducting important research on another rare form of cancer, Ocular Melanoma. The eye is the second most common site in the body for melanomas after the skin. The results of her review of the XXXXXXXXX Center’s database of cases involving metastic uveal melanoma from 1994 to 2004 are now being used to revise screening strategies and will have an impact on the future standard of care for patients with ocular melanoma. Further, the results of her research were presented at the International American Society of Clinical Oncology conference in Orlando, Florida in May 2005. The results of her research studies have provided major advances in the treatment and cure of cancers and malignancies.
